Reach for the Stars Form

During July and August, you can make a meaningful impact by fundraising for individuals with Intellectual or Developmental Disabilities (IDD). Through this initiative, 50% of all funds raised will directly support the IDD program of your choice within your community, while the remaining 50% of funds raised will benefit The Hometown Foundation’s IDD initiatives.

At The Hometown Foundation, we are dedicated to supporting IDD organizations and their members year-round. Our signature fundraising event, The Dream Ride Experience, celebrates inclusion, advocacy, and the incredible accomplishments of our Dream Riders - individuals with IDD or those in need.

This year The Dream Ride Experience will take place at The Farmington Polo Club on August, 22, 23, and 24.

Program Wrap-Up Details:

1. At the end of the campaign, each retailer will mail The Hometown Foundation, Inc. a check for 100% of proceeds raised during the fundraising campaign. All checks are to be made out to The Hometown Foundation, Inc. Please mail checks and/or any unsold hang tags to the mailing address below:
