The Hometown Foundation’s Hardship Continuity of Care Program (HCCP) is intended to help eligible applicants, based on verified need and available funding to maintain continuity of essential medical care. HCCP helps provide financial assistance for COBRA premiums, as approved by the Board of Directors. It places special emphasis on supporting families with dependents in the Intellectual and Developmental Disabilities (IDD) community, recognizing the unique and ongoing nature of their care needs.

*This initial application will be reviewed by The Hometown Foundation within 5-7 days of submitting. If you meet our initial guidelines, we will schedule a phone call to learn more about your circumstances. After that call you will be required to submit supporting documentation related to COBRA, description of medical conditions or ongoing treatments and other related documents.
